Students who choose to disclose their disability to receive accommodations must complete the Student Disability Survey and Release of Information forms which are available in the Office of Disability Support Services (DSS), or by selecting the links below. In addition, students must provide documentation of their disability. Because requirements vary according to disability, please contact our office for your individual documentation requirements.

• Disability Support Services Brochure An informational brochure designed to familiarize you with the Office of Disability Support Services.
• Student Disability Information Sheet A registration form needed to apply for DSS services.
• Release of Information A form which allows the DSS Office to access your medical records and document your disability.
• Student Checklist An informational brochure designed to give you a step checklist on applying for services and signing up for testing accommodations.
