NOTE: This form must be submitted 3 weeks prior to event. An event is not confirmed until it has been determined that technical needs can be met and the event is approved by the Technical Specialist.
ABSOLUTELY NO FOOD OR DRINKS ARE ALLOWED IN THE THEATRE OR IN THE SEATING AREA - THIS INCLUDES LUNCH BOXES AND UNOPENED CONTAINERS OF FOOD AND DRINKS AS WELL!

DO NOT submit this form unless your dates and times have been scheduled.
* By checking this box, you verify that you have already scheduled your event date with the appropriate GCCC personnel. All GCCC related events (including high school dual-enrolled events), must be scheduled with Elizabeth Kelly (x 3806). If an event is community related and or involves renting the theatre, dates must be scheduled with Beth Bennett (3808).
PLEASE NOTE: If the date, times or technical requirements of your event change, you are responsible for informing ALL of the proper people and confirming those times and dates. (the event scheduler and the technical director).

* The actual public activity for which the theatre is being used, exclusive of any set-up or preparation time.

Use of the following are not allowed:
For theatrical events please send a drawing of your stage setup. DO NOT SEND VIA COURIER!
If extra handicap needs please specify below.
For Example: number of wheelchairs/listening devises.
NOTE: These are always available in limited quantity, but if large numbers are required, please specify.
